In this Case Clinic, Bruce Bacon, MD, discusses the management of a 61-year-old woman with type 2 diabetes mellitus, hypertension, and an elevated body mass index who was unsuccessfully treated for hepatitis C virus (HCV) infection with an interferon-based regimen 15 years ago. Recent testing has revealed HCV genotype 1a and advanced fibrosis; she also is positive for NS5A resistance. The case discussion highlights how findings of resistance and advanced fibrosis affect management as well as the importance of post-treatment surveillance for hepatocellular carcinoma.
