African-American and Hispanic women represent 25% of all US women; however, the two groups accounted for 80% of the estimated total of new HIV diagnoses in women. Among Hispanic women/Latinas, more than 85% of the estimated HIV diagnoses are attributed to heterosexual contact. To understand the dynamics of HIV acquisition and transmission in Hispanic communities, and to underscore the importance of the full spectrum of HIV-prevention strategies, this case vignette features a 23-year-old Hispanic, heterosexual woman who reports multiple partners and alludes to working casually as a commercial sex worker. In this setting, it is critical for clinicians to consider partner dynamics and assumptions about the role of Hispanic women in their communities when implementing evidence-based prevention strategies including preexposure prophylaxis (PrEP).
