PROGRAM DESCRIPTION
While the initial focus of inflammatory bowel disease (IBD) management is on recognition and treatment, other issues that need to be considered include the prevention of disease complications and management of associated disorders. Long-term management needs to address the IBD itself, along with any extraintestinal manifestations of the disease, adverse effects of medication(s) and in some cases surgery, and a myriad of quality-of-life issues. In this activity, Dr. Millie Long outlines the crucial role of the multidisciplinary care team—comprising both primary and specialist clinicians—in long-term monitoring and ensuring that the patient’s overall health needs are addressed through a collaborative approach that is aligned to the primary care quality metrics identified by the Centers for Medicare & Medicaid Services.
EDUCATIONAL OBJECTIVES
At the conclusion of this activity, participants should be better able to:Assess patient management in alignment with established primary care quality metricsCommunicate effectively to ensure that patients are engaged in appropriate self-management strategies and that treatment plans meet clinical goals and satisfy patient preferences
